Registration

• Member (Full Conference) – $85.00
  OPHA Members who have paid their 2012 Membership dues.
• Member - One Day Only - April 5 or 6 – $65.00
  Members who only register for April 5th.
• Non member - One Day Only - April 5 or 6 – $75.00
  Individuals who are not OPHA Members and who will only attend on April 5th.
• Non member (Full Conference) – $125.00
  Individuals who are not members of OPHA.
• Oklahoma City-County HD Purchase Order
  Employees who work for the OCCHD and whose membership and conference registration is invoiced to OCCHD.
• OSDH Purchase Order
  OSDH employees who are current OPHA regular members may charge their member-registration fee to the OSDH Purchase order. OSDH will not pay the non-member registration fee. A cap has been set by OSDH. Once this cap is reached, no additional registrations will be charged to OSDH.
• Sponsor - Free
  Conference sponsors may select this type to register their allotted conference attendees.
• Student/Retiree Member - One Day Only - April 5 or 6 – $40.00
  Attendance on April 5th only
• Student/Retiree Member (Full Conference) – $50.00
  Students and Retirees who are OPHA members.
• Student/Retiree Non Member - One Day Only - April 5 or 6 – $55.00
  Full-time enrolled students and retirees of a public health agency who are not current members of OPHA and who only want to attend the conference on April 5th.
• Student/Retiree Non Member (Full Conference) – $70.00
  Full-time students and retirees who are not OPHA members.
• Tulsa City/County HD Purchase Order
  Employees of TCCHD whose membership and registration fee is invoiced to TCCHD.
• University of Central Oklahoma Purchase Order
  Employees and students of UCO whose membership and registration fee is invoiced to UCO.
